Clinical Case Manager - DHS (Santa Monica)
SUMMARY
In
partnership with the Department of Health (DHS), the Clinical Case Manager will
provide case management and clinical services to clients who are both currently
and formerly experiencing homelessness. The Clinical Case Manager will help
their clients to attain and retain permanent supportive housing in the
community.
The
Clinical Case Manager will work with clients to provide intensive field-based
case management and clinical services while employing a trauma-informed care
and harm reduction approach to a diverse group of clients with complex trauma
facing medical, mental health and substance use barriers. Client remains in the
program for as long as they require subsidized housing. For many individuals,
this may mean their entire lifetime.
ESSENTIAL DUTIES & RESPONSIBILITIES
•In collaboration with DHS, outreach to individuals who have chronic health conditions and are high utilizers of emergency county health services with the objective of an engaging individual to determine motivation and eligibility for permanent housing
•Complete pre-authorization and application package to submit to DHS per their policies and procedures
•Assist engaged members with homeless section 8 voucher to find a safe place to live and contact Property Managers and Landlords to set up appointments to show members different apartment options
•Travel to multiple housing locations in the greater Los Angeles area to provide case management; coordinate and implement mental health and recovery support services with individual members (caseload of -10-15)
•Coordinate with DHS to ensure members are connected to primary health care and reduce the need for emergency health care services
•Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health
•Collaborate with each member to develop their individual service plan; review and update it quarterly and upon completion of goals
•Assist management and other Clinicians with program evaluation and modification for future program improvements and growth
•Assist members with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of Lamp Community’s model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation
•Work cooperatively and cohesively with other members of the RIST team, including participation in weekly staff meetings, group clinical supervision, and training and provide clinical consultation to other RIST advocates as needed
•Develop and maintain a complete, accurate, and current member file with all required documents from DHS and HACLA
•Assure that all member paperwork and data collection is complete and up-to-date in the Homeless Management Information System and case management software
•Adhere to the National Association of Social Workers Code of ethics
QUALIFICATIONS
•MSW or MFT who is BBS eligible (Supervision hours are available for this position)
•Minimum of two years of experience working with homeless individuals, preferably those living with mental illness and or substance addictions, or an equivalent combination of education and experience
•Current, valid California Driver’s License with an acceptable driving record and reliable vehicle
•Able to work with minimum supervision
•Outstanding clinical and strong team-building skills
•Detail-oriented with excellent time management, organizational, written, verbal, and computer skills
•Able to obtain and maintain CPR/1st Aid certification
•Minimum 35 WPM typing speed preferred
•Bilingual in English/Spanish preferred
