Position Overview:
Manages a primary care panel of participants and provides continuity primary health care to PACE participants that reside in the community or long-term care setting with a focus on goal oriented, person-centered care, disease progression, disease prevention, wellness and management of chronic and episodic illness consistent with the participant’s goals of care. Works in close collaboration with all members of the PACE Interdisciplinary Team.

Establishes realistic goals and helps develop and manage individualized plans of care with the IDT team for program participants.

Provides primary health care to Summit ElderCare program participants focusing on disease prevention, wellness and management of chronic and episodic illness. Addresses advance directives in a participant-focused manner focusing on what matters most.

Works in close collaboration with the program’s interdisciplinary team to establish realistic goals and care plans and develop and manage individualized plans of care for program participants.

Our mission

Summit ElderCare is the only program of its kind in central Massachusetts. Summit ElderCare started as part of a PACE demonstration project, and is one of Eight PACE programs in the State. There are more than 118 PACE programs nationwide. And, although Fallon Community Health Plan sponsors Summit ElderCare, a participant does not have to be an FCHP member to enroll.

Our history

Established in 1995 as an alternative to nursing home placement, Summit ElderCare is focused on caring for older adults in an environment that helps enhance their quality of life and autonomy by helping them to stay in their homes and communities.

Recognizing a community need for more comprehensive services, care and support for frail elders who want to remain independent, and their caregivers, FCHP founded the Elder Service Plan, now known as Summit ElderCare. It started as part of a statewide project in Massachusetts under the leadership of the East Boston Neighborhood Health Center's PACE program. The program began with just eight participants. Since then, hundreds of participants and their caregivers have received services from the program.

Today, Summit ElderCare is one of six PACE programs in Massachusetts and more than 90 programs nationwide. This Program of All-inclusive Care for the Elderly (PACE) first became available to residents of Worcester County, the city of Marlborough and the town of Hudson in 1995. The program was provided permanent provider status by the Centers for Medicare & Medicaid Services in September 2002.

Qualifications

Education:

Master’s degree required. Graduate of an accredited Nurse Practioners program   

License:

Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing. Access to reliable transportation to carry out duties, both center-based and community, is required

Experience:

One to three years of Nurse Practitioner experience preferred with at least one year of experience in geriatric care.

Experience working in a healthcare setting as a member of a professional clinical team.

Experience with care coordination.
